  • Work Permit Applications

    The first step in obtaining a work permit is to complete the Work Permit Application (State of California Statement of Intent to Employ a Minor and Request for Work Permit Certificate of Age Form CDE Form B1-1(Rev 02-14) See the copy below. You will find them in the mailbox next to my office door already printed.

    1. Complete it in pen! It is a legal document.
    2. Make sure you have included your Social Security Number and ID # on the form.
    3. Make sure it is completed fully and "legible".
    4. Return the completed application to my office and clip it to my door face down. If you drop it off in the morning and I am on campus, it will be ready for you to pick up at lunch the same day.
