Classified Employees

PSUSD provides vision coverage for employees enrolled in a District medical plan through VSP, administered by Northwest Administrators, Inc. (NWA). The VSP plan offers professional vision care and high-quality lenses and frames through a broad network of optical specialists. You’ll receive the most benefits when using a network provider. If you choose a non-network provider, you will need to pay all charges at the time of your appointment and submit an itemized claim to VSP. See the plan summary below for complete details.

Life and Accidental Death and Dismemberment (Life/AD&D) coverage through Northwest Administrators, Inc. (NWA) is provided only to classified full-time active employees. This coverage includes $30,000 in life insurance provided by the Teamsters Miscellaneous Security Trust Fund through Prudential.

Benefits are paid to the beneficiary you name on the Teamsters Beneficiary Designation Form on file with NWA. This form can be found below, or it can be requested by contacting Northwest Administrators at (877) 214-8928 or downloaded from www.nwadmin.com. Your beneficiary designation becomes effective once it is received by NWA and may be updated at any time by submitting a new form.

PSUSD offers you the opportunity to purchase MetLaw Legal Services through Hyatt Legal Plan with after-tax dollars at discounted group rates. This plan provides coverage for a number of legal matters such as will preparation, buying or selling a primary home, document review, civil litigation defense by telephone and office consultations for numerous matters (except employment related), business or pre-existing matters. In addition to the District-paid Basic Term Life and AD&D coverage, classified full-time employees may choose to purchase additional Term Life and Accidental Death and Dismemberment (AD&D) insurance at discounted group rates through MetLife.

If you or your spouse request coverage above the Guaranteed Issue amount, you will be asked to complete a Statement of Health Questionnaire, which is subject to insurance carrier approval. Coverage may be approved or declined based on a review of your health history.

If you lose your district-sponsored health coverage due to a qualifying event, you may be eligible to continue your benefits through COBRA. COBRA allows you and your covered dependents to keep medical, dental, and/or vision coverage for a limited period of time.

Under COBRA, you are responsible for the full monthly premium, which may be higher than what you paid as an active employee. Coverage is generally available for up to 18 months, depending on the qualifying event.

For Classified/Teamsters employees, medical and vision COBRA enrollment is handled directly through Northwest Administrators (NWA).
